Jeffery Lyth is a safety practitioner, author and advisor with more than 30 years of experience working in complex, high-hazard environments across Canada and internationally. His career spans emergency response, major construction and infrastructure projects, large-scale events, and senior advisory roles supporting industry, regulators, and executive leadership.

Jeff is the author of The 4Ds for HOP and Learning Teams and is widely known for his practical, experience-based approach to Human and Organizational Performance. His work focuses on helping organizations improve organizational learning, system understanding, and leadership capability grounded in how work is actually done.

He is the North American principal of Learning Teams Inc. and works closely with organizations to help them help themselves as they integrate contemporary safety practices into everyday operations. His current practice includes the ethical use of HOPCopilot AI tools to support learning without replacing human judgment.
